Our AthenaHealth Patient Portal is a secure way for the midwives and staff to communicate with you. 

Through the portal, you can view lab results, update your medical history, and even directly correspond with the midwives. Unfortunately, we cannot accept payments or make appointments through the portal at this time - please call the office at 708.848.3800. 

If you are a new patient, upon giving us your email address when you make your appointment, you will receive an invitation to the portal. Please log in prior to your appointment to fill out your medical history. It makes your appointment far more efficient and gives you ample time to consider your history, so we have the most accurate vision of your health!  

For communication with our office through the portal, expect a delay in receiving a reply, as our software must first direct us your inquiry. If you have a time-sensitive question, please call the office directly at 708.848.3800. If you are having an emergency, please call 911.


If you are transferring to our practice or transferring out of our practice, please submit our Medical Release Forms.


Transfer of care obstetric patients must bring their medical records to their first appointment. We need your records in order to provide you with care! Please fill out our medical release form for your previous provider a few weeks in advance if possible, and be sure to follow up that they received it and will have your records sent to us before your appointment date. 

If you are having trouble getting records from your previous provider, please call our office at 708.848.3800, as our nursing staff may be able to assist you. 

To request your records from our office, you may fax forms to us at 708.848.0008.
